Work around lack of support for UNICODE BOM in UTF-8; it was added in XML 1.0
Second Edition, but was missing from the original XML 1.0 Rec. Many thanks to
Peter K. Sheerin <pete@petesguide.com> for catching this (and hounding me until
I got around to fixing it! ;D), and Masayasu Ishikawa <mimasa@w3.org> for
clarifying the issue.
